Visitors Expectations Fact Sheet Revised 9/17/2020


  • All visitors must present documentation of a negative Covid test within the last 7 days prior to visit. If no documentation presented you will not be able to visit. Only exception is an end of life situation.
  • All visitors will be screened for signs & symptoms of Covid-19 prior to resident access. Visitation will be refused if an individual (s) exhibits any symptoms or do not pass the screening questions.
  • Screening will consist of both temperature checks & screening questions to assess potential exposure to Covid-19. The screening questions include international travel or travel to other states designated under the Commissioner’s travel advisory, symptoms & possible exposure.
  • Documentation must include the following for each visitor: first & last name of visitor, physical street address of visitor, daytime & evening phone number, date & time of visit and email address if available.
  • Visitors must wear a facemask or face covering which covers nose & mouth at all times.
  • Hand sanitizer to be used before & after visit.
  • Remember to maintain social distancing of at least 6 feet.
  • Visitation will be limited to outdoor areas in front of building weather permitting. Under certain limited circumstances visitation will be inside in our main dining room with no more than 10 individuals (including patient & staff) who are socially distanced (6 ft) & wearing a facemask or face covering at all times. Visits will be 30 minutes in duration. Please be mindful visitation is limited & visitors must be prompt otherwise the visit will have to be rescheduled.
  • No more than 10% of the residents shall have visitors & only 2 visitors will be allowed per resident at any one time. For example, if census is 150 then 15 residents can have visitors or if we are at full census at 180 then 18 residents can have visitors per day.
  • Visitor under 18 years old must be accompanied by an adult.
  • At this time, visitation is strictly prohibited in resident rooms or care areas with the exception of end of life visits.
  • Visitation is by appointment only. You must contact Recreation Department to set-up appointment at extension 1070 or 1071. A copy of our visitation plan is posted on our website & visitation updates will be broadcasted via robocall from Administrator.
  • Staff is required to monitor the visitation process. If any visitor fails to adhere to the protocol, he/she/they will be prohibited from visiting for the duration of the Covid-19 state declared public health emergency.
